Instructions for accessing the AOS CME Evaluation Form

Thank you for attending the recent Spring meeting.

To receive your CME credit and certificate of completion, please complete the evaluation form by following the steps below.

1. Read the email sent to all registered attendees and click on the link to access the form. 
   
   

2. Click or type the link into your browser: 
   

3. Enter all required information (including your full name, email address, number of hours, ACS and/or ABOHNS ID (optional).

4. Complete all evaluation questions.

5. Click FINISH at the bottom of the form.

6. After completion, you will receive a confirmation email with a pdf of your CME certificate of completion.

IMPORTANT NOTES

• The CME evaluation must be completed before the deadline.(TBA)

• CME credit can only be issued after the evaluation form is submitted and within 30 days of the meeting.
